2023 – 2024 Credit Card Form

I give permission to have this credit card charged, by ‘Miss Tanya’s School of Dance’, if my tuition, recital fee, recital costume deposit, recital costume balance payments, competition balances or any miscellaneous fees are not paid by the set due date(s) or if my account is 10 days delinquent. I also understand that if a new month begins and my dancer decides to no longer participate in a class/classes that I am still responsible for that month’s tuition. Withdrawal from a class must be done in writing or email before the new month begins. It is my responsibility to inform the faculty of any changes to my child’s dance schedule by the 25th of the prior month. I also understand that if my tuition payment is 10 days negligent and my credit card, on file, was declined, that my child may be removed from class until the balance is settled. All payments received will be applied to the oldest payment due. I recognize that June 2024 Tuition is due by May 20th,2024 and if I have not paid it in full by this designated date my credit card will be charged. My online payment is my indication that I have read, understand and agree to comply with all these policies.
