Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19

The coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. Federal, state, and local governments and federal and state health agencies recommend students of all ages are vaccinated. 

Miss Tanya’s School of Dance Inc. has put in place preventative measures to reduce the spread of COVID-19, by adding the following:

1.  We will follow the CDC guidelines for cleaning and sanitization. Hand sanitizer is located throughout the studio. We have purchased the correct cleaning products recommended by the CDC and are EPA approved for all hard surfaces, bathrooms and also for our Marley floors and will clean the studios surfaces at the end of each day. 
2. We have H13 HEPA Air purifiers for all three studios and one in the lobby area.
3. We have decided to keep the wait areas closed for the 2023-2024 season, as we have noticed that there is less disruption in the classroom. Parents/Guardians are allowed to come into the studio with students that are in classes ages 4-6 years for the first couple weeks to get the student settled into their classroom. Once they are in the classroom, we kindly ask that you exit the studio. Parents/Guardians are allowed in the studio to make payments, but must wait until all students are in their assigned classroom. 
4. We will use the studio cubbies for all personal belongings 
5. Students may bring in their own water (ONLY) labeled clearly with their name. All water coolers have been removed from the classrooms. The NO FOOD policy will be strictly enforced. 
6.  The changing room will not be used.  Students will be required to come in full attire.
7. All students will enter and leave through the main entrance of the studio. 

Even with all of the above precautions, Miss Tanya’ School of Dance Inc. cannot guarantee that you or your child/children will not become infected with COVID-19. 

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child/children and I may be exposed to or infected by COVID-19 by attending Miss Tanya’s School of Dance Inc. and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at Miss Tanya’s School of Dance Inc. is not from the actions or negligence of the studio owners, staff and others, including, but not limited to, Miss Tanya’s School of Dance Inc. employees, volunteers, and program participants and their families.

I agree to assume all of the risks and accept responsibility for any injury to my child/children or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child/children may experience or incur in connection with attending classes at Miss Tanya’s School of dance Inc. I hereby release, covenant not to sue, discharge, and hold harmless Miss Tanya’s School of Dance Inc., its owners, employees and representatives, of and from the claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of from Covid -19, whether a COVID-19 infection occurs before, during, or after participation in any Miss Tanya’s School of Dance Inc. program, class or event.

My child or I have not in the last  (5) days had any fever, cough, fatigue or other symptoms commonly associated with COVID-19:

My child or I have not been in contact with any family member or social acquaintance that has had any fever, cough, fatigue, or other symptoms commonly associated with COVID-19 or have traveled to arears that have a rise in Covid 19 infections.
