I give permission to have this credit card charged, by ‘Miss Tanya’s School of Dance’, if my tuition, recital costume deposit, recital costume balance payments, competition balances or any miscellaneous fees are not paid by the set due date(s) or if my account is 15 days delinquent. I also understand that if a new month begins and my dancer decides to no longer participate in a class or the program that I am still responsible for that month’s tuition.  Withdrawal from a class must be done in writing before the new month begins. It is my responsibility to inform the faculty of any changes to my child’s dance schedule by the 25^th of the prior month. I also understand that if my tuition payment is 30 days negligent and my credit card, on file, was declined, that my child may be removed from class until the balance is settled. I recognize that June 2022 Tuition is due by May 22nd  2023 (not June 15^th 2022) and if I have not paid it in full by this designated date my credit card will be charged. My signature is my indication that I have read, understand and agree to comply with all these policies.
